apply for a citizencard
- Why apply for a CitizenCard?
Many organisations need to be sure of your age or identity. Using a CitizenCard is voluntary, but it can make life much easier. Even if you have a passport or driving licence, CitizenCard means you can leave these more valuable documents safely at home. Using a CitizenCard means that you can access the products and services to which you are entitled.

- How can I apply?

1. Download an application form from the button above or here (PDF format, which requires the free Adobe Reader to open, if you don't already have it, you can get it here)
2. Or you can collect one from most supermarkets, convenience stores, off licenses, post offices or newsagents.
3. Complete the form and send it to CitizenCard with your payment.
PLEASE NOTE: All card applications should be sent to our Chester address:
CitizenCard, PO Box 3582, Chester CH1 9HY
Please
DO NOT send applications to the London address as they may be delayed.
- I have other questions -where can I find out more?
Most questions you might have will be covered on our Frequently Asked Questions page.
