You should notify us immediately if your card is lost or stolen. Someone who finds your card may attempt to use it for their own purposes.
If your original card was issued BEFORE March 2009, you will need to re-apply for a new card as normal. If you need an application form to reapply you can download one from our website or we can send you one by post (please provide your full name and address by email).
If your card has been issued SINCE March 2009, you will need to send in a letter requesting a replacement card. Clearly state your name and date of birth so that we can locate you on our database. You need to include a new photograph which does not need to be signed by the referee. You will also need to supply your current email address and mobile number. Please note if you have changed address you will need to supply both your old and new address when sending your request along with proof of your new address in the form of a utility bill, bank statement or council tax letter.
The Standard replacement card will cost £15 and take up to 28 days to deliver, or you can apply for an Urgent replacement card which will cost £30 and take 2-3 working days to process once we have received your application by post.
